Care Coordinator - Live In
Help Someone Get From "What Now?" to "What's Next." -- And Be There Along the Way.
Work Environment: On-Site | Residential Co-Living Home
Schedule: Live-In Position | Scheduled Duties and Hours Based on Program Needs
More Than a Job. A Chance to Be Part of Someone's New Beginning.
A hospital discharge isn't always the end of someone's healthcare journey. Sometimes, it's the beginning of an even more important one.
Where will I recover?
Who can help me navigate my next steps?
Where am I going to live?
How do I build a more stable future?
At Anchor Co-Living, we believe recovery and stability begin with more than a roof over someone's head. They begin with compassion, consistency, connection, and having the right support at the right time.
We're looking for a compassionate, responsible, and highly organized Live-In Care Coordinator who wants to make a meaningful difference in the lives of individuals recovering from hospitalization, experiencing housing instability, or navigating complex healthcare needs.
This is a unique opportunity to live within one of our residential homes while helping create an environment where members feel supported, respected, and empowered to move forward.
You'll combine clinical knowledge, care coordination, housing navigation, and everyday resident support to help members move from recovery toward independence.
You won't just coordinate care. You'll be a consistent, trusted presence in a place where recovery and new beginnings happen.
What You'll Actually Do
Think of yourself as part care coordinator, part advocate, part problem-solver, and part steady hand. Because you'll be based within the home, you'll have the opportunity to build meaningful relationships with residents, recognize challenges early, and help create a positive and supportive living environment.
Support Recovery With Compassion
- Conduct and document daily member wellness check-ins.
- Monitor residents' general well-being and identify concerns requiring clinical follow-up.
- Coordinate medical appointments, transportation, medications-related follow-up, and access to essential services.
- Communicate with healthcare providers, hospitals, case managers, and managed care plans.
- Support residents in understanding and following their established care plans.
- Escalate medical or safety concerns appropriately and within clinical scope.
Because sometimes the most important part of recovery is knowing someone is paying attention.
Help Make the House Feel Like a Home
- Maintain a welcoming, respectful, and supportive residential environment.
- Serve as an accessible point of contact for residents during scheduled working hours.
- Help residents navigate daily challenges and communicate concerns.
- Support healthy roommate relationships, shared-living expectations, and conflict resolution.
- Coordinate basic resident needs, including meals, transportation, and community resources.
- Identify and report safety, maintenance, or operational concerns.
- Collaborate with property management and program leadership to help the home operate smoothly.
A stable home is about more than four walls. It's about creating an environment where people can focus on moving forward.
Turn Housing Goals Into Real Progress
- Help residents identify housing opportunities and available resources.
- Assist with housing applications, referrals, and required documentation.
- Connect residents with affordable housing, transitional housing, room-share opportunities, and assistance programs.
- Support budgeting, organization, household management, and independent-living skills.
- Help residents build healthy communication skills with landlords, roommates, and community partners.
- Identify potential barriers to housing stability and work proactively toward solutions.
Getting housed is a milestone. Staying housed is the goal.
Connect Healthcare, Housing & Community Support
- Coordinate medical, behavioral health, and social services.
- Collaborate with providers, MCPs, case managers, and community organizations.
- Develop and maintain individualized care plans.
- Help members identify measurable health, housing, and personal goals.
- Track progress, coordinate referrals, and follow up on services.
- Support smooth transitions between Recuperative Care, Housing Navigation, Housing Tenancy & Sustaining Services, and Enhanced Care Management, as applicable.
Your role isn't simply to make a referral. It's to help make sure the connection happens.
Keep Care Organized & Documentation Audit-Ready
- Complete daily check-ins and required documentation in the EMR.
- Maintain accurate care plans, member records, and service notes.
- Track authorizations, deadlines, and required renewals.
- Ensure documentation aligns with CalAIM, MCP, and organizational requirements.
- Communicate member progress, concerns, and important updates to leadership.
- Maintain complete, timely, and organized records.
Because compassionate care and excellent documentation go hand in hand.
Who Thrives in This Role?
We're looking for someone who understands that living where you work requires maturity, professionalism, healthy boundaries, and a genuine passion for helping others.
You're someone who:
- Leads with empathy but understands the importance of accountability.
- Creates a calm, respectful, and supportive environment.
- Builds trust while maintaining professional boundaries.
- Can handle difficult conversations with patience and confidence.
- Notices when someone needs additional support.
- Takes initiative and follows through without constant reminders.
- Is comfortable working independently and collaborating with a larger care team.
- Understands that living on-site does not replace established schedules, reporting procedures, or emergency protocols.
And most importantly, you believe that people deserve to be treated with dignity, especially during life's most difficult transitions.

- LVN or higher clinical credential
- Experience in care coordination, case management, community health, or related services
- Strong written and verbal communication skills
- Experience using electronic medical record (EMR) systems

- Ability to support individuals with complex healthcare and housing needs
- Strong problem-solving and conflict-resolution skills
