Short-Term Post-Hospitalization & Recuperative Care

The Role

We are seeking a Housing Care Manager to oversee resident support and documentation for individuals placed in Short-Term Post-Hospitalization and Recuperative Care housing.

This is a hands-on coordination role that requires both:

  • Strong relationship-building skills
  • High-level documentation accuracy

You will guide residents through a critical recovery and housing transition period while ensuring all services are properly documented in our Electronic Health Record (EHR) system.

About the Residents

Our residents are:

  • Medically stable but not ready to return to independent living
  • Recovering from hospitalization, illness, injury, or acute medical events
  • Often facing housing instability and limited support systems

They require a safe place to stabilize while planning their next housing step. Your role ensures that time is used intentionally and documented accurately.


Core Areas of Responsibility

1. Resident Support & Coordination

  • Conduct consistent resident check-ins
  • Assess needs, goals, and recovery progress
  • Support stabilization after hospitalization
  • Coordinate housing-related activities
  • Assist with discharge planning and transitions
  • Communicate with house staff and external partners

You are the central point ensuring each resident's stay is purposeful and progressing.

2. Full Ownership of Documentation

This role requires complete accountability for housing documentation.

You will:

  • Complete daily housing notes for 100% of active residents
  • Ensure documentation reflects engagement and services provided
  • Align notes with authorized services and approved length of stay
  • Maintain organized, audit-ready resident charts
  • Prevent documentation backlogs through daily completion

Accurate documentation directly protects billing, compliance, and program integrity.

3. Housing Navigation (When Authorized)

When residents are approved for Housing Navigation services, you will:

  • Support housing searches and application processes
  • Coordinate with landlords, housing providers, and community resources
  • Track search efforts, applications, and placement progress
  • Complete required monthly Housing Navigation documentation
  • Document outcomes clearly and consistently

Services must match authorization at all times.

4. Compliance & Quality Assurance

  • Monitor resident length of stay
  • Support timely and appropriate discharges
  • Respond quickly to documentation feedback
  • Follow established workflows and templates
  • Maintain charts in review-ready condition at all times

Performance Standards

Success in this role means:

Documentation

  • 100% of daily notes completed on time
  • Zero rolling documentation backlog
  • High consistency and accuracy across all entries
  • Prompt correction of any documentation issues

Program Integrity

  • No billing delays due to missing documentation
  • Services delivered only as authorized
  • Length of stay aligned with program limits
  • Discharge documentation completed promptly

Housing Navigation

  • Monthly documentation completed on time
  • Clear tracking of housing progress and outcomes
