Anchor Co-Living | CalAIM Program

Anchor Co-Living is seeking a Lead Care Manager to support Medi-Cal members through CalAIM by providing care coordination and housing navigation services.

Key Responsibilities

• Assess member medical, behavioral, social, and housing needs
• Create and manage individualized care plans
• Coordinate healthcare, social services, and housing supports
• Assist with housing applications, move-ins, and tenancy support
• Build relationships with landlords and housing agencies
• Maintain compliant documentation and reporting

Qualifications

• High school diploma or GED required; degree preferred
• 1+ year experience in care coordination, case management, or housing navigation
• Familiarity with CalAIM, Medi-Cal, and housing systems preferred
• Strong communication, documentation, and advocacy skills
• Valid CA driver's license and reliable transportation

Compensation

• $21-$25 per hour, based on experience

Work Environment

• Hybrid role with remote, and in-person member visits
