Anchor House is seeking a Lead Care Manager to support high-need Medi-Cal members through the CalAIM Enhanced Care Management (ECM) program. This role focuses on overseeing care coordination, supporting care managers, and ensuring quality service delivery.

Responsibilities

• Lead and support care managers to ensure effective care coordination
• Oversee assessments, care planning, and service coordination under CalAIM ECM
• Collaborate with the ECM Program Manager and external providers
• Monitor quality measures and reporting requirements
• Identify program improvements and communicate updates to the team

Qualifications

• 3+ years of relevant experience required
• High school diploma or GED required
• Knowledge of Medi-Cal, CalAIM, or community services preferred
• Strong communication and organizational skills
• Proficiency with Google Workspace and basic office tools
• Flexible availability, including occasional evenings or weekends
• Multilingual skills a plus

Compensation & Schedule

• $18-$22 per hour
• Full-time, Monday-Friday (on-call/weekend opportunities)
• Paid time off
